2. Choose to dispense manually or using eDispense
3. Will you need a dispensing cabinet?
4. Fill out the new account packet and deliver with:
   • Executed quote
   • Valid and legible copy of medical license for formularies with legend drugs only or, for formularies that contain controlled substances (CII, III IV & V)
   • A valid and legible copy of DEA license with address that matches ship to address is MANDATORY
   • New Account and Order is processed and shipped within 3-5 business days

• Cash & Carry
• 2% 20 / Net 30
• $5,000.00 revolving credit (increased credit OAC)
• Orders over $250.00 - Free Shipping (UPS Ground)
• $10.00 shipping charge on all orders under $250.00 (UPS Ground)
